Organ, tissue, and eye donation can save 7 lives, restore sight to two people, and help many more. Almost anyone can be a donor at the time of death, regardless of their age or medical condition. (Everyone thinks they are too old or too sick, but healthy organs have come from donors in their seventies and those who suffered from many types of illness.)

Carefully choose an option that best suits you:

Option 1 - I would like to be a donor, and I live in the state of Virginia.

Option 2 - I would like to be a donor, but DO NOT live in the state of Virginia. (save7lives.org currently records the gifts of resident Virginians only.)

Option 3 - I am already registered as a donor through the DMV. (You will need to create an account using your existing DMV information.)

Option 4 - I would prefer to fill out and mail a traditional form instead of using the online form. (Download and print the PDF format.)

Option 5 - I have already completed this form and wish to modify my records.
